They are able to educate you about your prognosis and empower you to make more informed healthcare decisions. They also provide you with a single point of contact when you have questions or concerns regarding a healthcare issue. They can also help coordinate tests, treatment options, timely referrals to specialists and other ancillary care providers, based on your needs and goals.
